How to make a FOIA request:

If you would like to obtain information from a public body, you should begin by writing down a list of the information you are seeking. Then email a FOIA request, or prepare a letter to our FOIA officer.

Your request should include your name, address, the date and a daytime phone number so that we may contact you if we have questions. Please describe the information you need with sufficient detail so that Mid-State Special Education can find the requested records.  Mid-State Special Education requires that FOIA requests be in written form of either email or letter.
